Resumo: Histopathological analysis is a key diagnostic tool in the evaluation of suspected Sjögrens syndrome. A female patient with severe xerostomia, positive anti-Ro/SSA and anti-La/SSB serology, and a history of rheumatoid arthritis was referred for minor salivary gland biopsy. During the procedure, the dental surgeon identified a surgical bed with scant salivary glands. Histological examination revealed skeletal muscle and connective tissue containing spaces partially filled with homogeneous, pale basophilic material on hematoxylin and eosin staining, strongly highlighted by Alcian Blue. No salivary gland tissue was identified, precluding diagnostic assessment. The distribution and staining profile were consistent with hyaluronic acid, commonly used as a dermal filler. The patient remains under follow-up. These findings underscore the importance of clinicopathological correlation to avoid misinterpretation and ensure accurate diagnosis in salivary gland disorders.
