Título: DIAGNOSTIC CHALLENGE OF A PIGMENTED PALATAL LESION: A CASE REPORT

Resumo: Oral pigmented lesions include melanocytic nevi, melanoacanthoma, and melanoma, a malignant neoplasm with poor prognosis that may mimic other lesions, making histopathologic diagnosis through biopsy essential.
A specimen was received from a 43-year-old female patient presenting with a macule on the palate, showing black and brown coloration. An initial diagnosis of melanoacanthoma was made, however, subsequent enlargement prompted biopsy.
Histopathologic examination revealed a pigmented neoplasm involving the basal layer, with cellular pleomorphism, nuclear hyperchromatism and subepithelial melanin deposition. Immunohistochemistry was positive for Melan-A, Ki-67, S-100, SOX10 and HMB-45, supporting a final diagnosis of oral melanoma.
Timely biopsy of oral pigmented lesions aims to rule out malignancy. Adequate deep incisional sampling from the area with the most significant clinical changes, allows for better evaluation. Suboptimal samples may compromise diagnosis, while adjunctive techniques increase diagnostic accuracy. The patient was referred to oncology.
