Resumo: A 33-year-old female patient was referred to the oral medicine service with a swelling in the anterior maxilla, with an evolution of approximately 15 years. Extraoral examination revealed facial asymmetry associated with swelling in the right anterior maxilla. Intraoral examination showed a tumor, normochromic, with a hard consistency and well-defined borders between teeth 13 and 14. Computed tomography demonstrated a well-defined multilocular hypodense lesion, with cortical thinning, buccal cortical perforation, and root resorption of the involved teeth. Initial diagnostic hypotheses included ameloblastoma and odontogenic myxoma. Needle aspiration was negative. Incisional biopsy revealed an odontogenic neoplasm composed of cuboidal to columnar epithelial cells with hyperchromatic nuclei and reverse polarity, areas resembling stellate reticulum, as well as dentin/osteodentin formation and ghost cells. The final diagnosis was a dentinogenic ghost cell tumor. The patient declined surgical treatment.
