Resumo: A 19-year-old female patient presented with a gingival lesion, without bone involvement. The patient reported discomfort in the gingiva of tooth 36; she observed that the nodule increased in size over time. The lesion was painless, and she sought treatment after two years of growth. The diagnostic hypothesis was traumatic fibroma. An excisional biopsy was performed. Macroscopic analysis revealed a fragment of soft tissue measuring 20 × 12 × 9 mm, with a rounded shape, and gelatinous consistency. Microscopic analysis revealed a non-encapsulated lesion composed of myxoid tissue, with the presence of short and delicate collagen fibers. The cells were variably spindle-shaped, rounded, or stellate, interspersed with delicate blood vessels. Few cells showed positive staining for S100. Based on the clinical and microscopic features, the diagnosis was focal oral mucinosis. The patient remains under follow-up, with no signs of recurrence.
