Título: CONGENITAL FACIAL INFILTRATING LIPOMATOSIS: A CASE REPORT

Resumo: A 14-year-old boy presented an asymptomatic enlargement of the right hemiface, extending from the upper lip to the midface since birth and associated with aesthetic complaints. Intraoral examination revealed diffuse fibroelastic swelling of the buccal mucosa and upper lip limited by the midline. Dental crowding and maxillomandibular discrepancy were observed. The patient's mother denied systemic conditions. Suspecting lymphatic or vascular malformations, Doppler ultrasound was performed, revealing a poorly defined solid tissue with normal vascularization in the right upper lip, ruling out vascular lesions. Incisional lip biopsy was conducted, and histopathological evaluation showed fibrous tissue containing mature adipocytes in deep areas. Based on clinical history and imaging findings, a diagnosis of congenital infiltrating lipomatosis of the face (CIL-F) was established. CIL-F is characterized by diffuse proliferation of mature, non-encapsulated adipose tissue infiltrating adjacent structures, leading to facial asymmetry. Treatment is mainly surgical, especially for aesthetic purposes. Recurrence is common.
