Resumo: Primary intraosseous carcinoma not otherwise specified (PIOC-NOS) is a rare and
aggressive malignancy of the jaws marked by poor prognosis and low survival rates.
This is a case report of an 83yo male complaining of severe pain in the left side of his
face, trismus and dysphagia noticed after a tooth extraction performed 3 months earlier.
Clinically, a mandibular swelling and unhealed dental socket of tooth 37 covered by
fibrinopurulent membrane were observed. Cone-beam computed tomography showed a
hypodense lesion with diffuse margins and "moth-eaten" appearance involving the
mandibular body, as well as cortical expansion and erosion. Histopathological
examination of an incisional biopsy showed sheets, nests, and cords of malignant
epithelial cells exhibiting intense atypia, perineural and muscle invasion, confirming
PIOC-NOS as diagnosis. Patients health condition was critical and worsened rapidly.
Pain control was performed with opioids. The patient died 30 days after diagnosis
shortly before starting chemotherapy and radiotherapy.
