Resumo: Ointment pseudo-cheilitis (OPC) is a rare condition characterized by adherent crusts on the lips composed of dried saliva and residues of topical medications, without evidence of true lip inflammation. Clinically, patients report pain and excessive concern about the lesions. Microscopy shows a normal to slightly hyperplastic epithelium with minimal inflammatory infiltrate. To date, about 14 OPC cases have been reported. Management involves patient reassurance, gentle removal of crusts after saline soaking, and, in some cases, antidepressant therapy. A 58-year-old female patient presented with patches and plaques restricted to the upper and lower lips 30-day ago. She reported that the onset of the condition coincided with emotional distress and the use of topical ointment. The patient denied smoking, alcoholism, exposure to extreme temperatures, and occupational contact with toxic chemical compounds. Microscopy revealed epithelial hyperplasia, melanin pigmentation in basal keratinocytes, and mild inflammatory infiltrate. After removing the topical ointment, the lesions resolved.
