Título: RADIOMICS ANALYSIS IN IRRADIATED HEAD AND NECK TISSUES: A METHODOLOGICAL FRAMEWORK FOR STANDARDIZATION

Resumo: Objective: To propose a standardized radiomic workflow for analyzing irradiated head and neck tissues, addressing technical challenges in feature extraction from Cone-Beam Computed Tomography scans after radiotherapy.
Study Design: This methodological study outlines a robust six-step analysis based on the Image Biomarker Standardization Initiative (IBSI). Images were acquired using a standardized protocol (i-CAT Classic, 120 kVp, 37.07 mAs, 0.2 mm voxel size). The workflow includes: 1) acquisition parameter control; 2) pre-processing (0.2 mm³ isotropic resampling and gray-level discretization); 3) expert-guided segmentation of volumes of interest; 4) feature extraction (morphology, intensity, and texture); 5) stability assessment; and 6) dimensionality reduction to avoid overfitting.
Results: Critical technical challenges identified include metallic artifacts and dose-dependent density changes. Isotropic voxel resampling (0.2 mm³) and fixed-bin-size discretization are important strategies to improve texture feature reproducibility per IBSI guidelines. Consensus-based segmentation is relevant to reduce uncertainty in regions with altered anatomical landmarks, providing a stable basis for feature extraction.
Conclusion: A standardized radiomic workflow supports developing a reproducible "virtual biopsy" for irradiated tissues. Methodological rigor in acquisition, segmentation, and processing is essential for future studies investigating imaging biomarkers of radiotherapy-induced toxicities.
