Resumo: Objective: To characterize the profile of patients with malignant neoplasms of the oral and maxillofacial region diagnosed at an Oral Medicine Service in Rio de Janeiro between 1987 and 2025. Study design: Observational, descriptive, cross-sectional, and retrospective study, based on medical records and histopathological reports. Sociodemographic, behavioral risk factors, clinical, imaging and histopathological variables were collected and analyzed using descriptive statistics with frequency distribution and temporal stratification. Results: A total of 662 patients were included, corresponding to 3.5% of the patients seen during the period, predominantly males, with a median age of 59 years, white, retirees, and residents of Rio de Janeiro. Hypertension was the most frequent comorbidity and most patients were smokers (1-2 packs/day) and alcohol consumers. Lesions were mainly ulcers and nodules/papules in the anterior two-thirds of the tongue, reddish or red-white coloration, ulcerated surface, sessile base, raised borders, irregular contours, and associated pain, with evolution up to 6 months and size ≤3 cm most frequent. Squamous cell carcinoma was the most frequent diagnosis (75%), followed by salivary gland tumors and lymphomas. Conclusion: The extensive historical series constitutes a strength, allowing a broader view of patients with cancer diagnosed in a reference university Oral Medicine Service.
