Resumo: Assessment of immune and nutritional status has emerged as an important component in prognostic stratification, with the Prognostic Nutritional Index (PNI), derived from lymphocyte count and serum albumin levels, standing out as a relevant marker. This study aimed to analyze the ability of the PNI to predict postoperative complications and overall survival in individuals with head and neck cancer (HNC) undergoing surgery. A retrospective cohort was conducted at an oncology referral center, between 2016 and 2022, including 69 consecutively enrolled subjects aged 18 years or older with histologically confirmed HNC. The PNI was calculated using the formula 10 × albumin (g/dL) + 0.005 × total lymphocyte count (/mm³), with a cutoff value of <50 indicating nutritional risk. Postoperative complications were classified according to the Clavien-Dindo system. Individuals with PNI <50 had a higher incidence of complications (52%) compared with those with PNI ≥50 (33.3%) (p=0.02). Median overall survival was 72 months in the PNI ≥50 group and 21 months in the PNI <50 group (p=0.0273). In conclusion, the PNI is an accessible, low-cost, and clinically applicable tool for predicting surgical complications and estimating prognosis in HNC, underscoring the importance of early nutritional assessment in oncologic care.
