Resumo: Actinic cheilitis is a chronic potentially malignant disorder associated with cumulative ultraviolet exposure that predominantly affects the lower lip. A 49-year-old male patient presented with loss of definition of the lower lip vermilion, with unknown duration. Clinical examination revealed areas of epithelial atrophy and surface alteration suggestive of actinic damage. An incisional biopsy was performed to evaluate the degree of epithelial dysplasia. Histopathological examination revealed actinic cheilitis with severe epithelial dysplasia. Considering the risk of malignant transformation, the patient received counseling regarding photoprotection and was referred to an oral and maxillofacial surgeon for surgical management with vermilionectomy. The patient remains under clinical follow-up.
