Resumo: Intraoral sebaceous carcinoma (ISC) is an exceedingly rare neoplasm, with approximately 14 cases documented in the literature, primarily affecting white males aged 50-70. We report a case involving a 44-year-old white female with no traditional risk factors for oral cancer. Clinical examination revealed a 2 cm ulcer on the dorsum of the tongue with indurated borders and a necrotic center; no regional lymphadenopathy was detected. Exfoliative cytology suggested malignancy. Subsequent histopathological analysis revealed an epithelial transition to a biphasic area featuring spindle-shaped cells, anisokaryosis, and neoplastic multinucleated giant cells, consistent with ISC. Immunohistochemistry was p40-positive and p16-negative. The patient was referred to an oncology center and is currently undergoing antineoplastic treatment. Management includes photobiomodulation and pharmacological therapy to mitigate treatment-induced oral complications. This case highlights the importance of considering rare entities in atypical demographic profiles.
