Resumo: Objective: To compare the impact of two photobiomodulation (PBM) protocols-intraoral (IOPBM) and defocused extraoral (EOPBM)-on oral mucositis (OM) severity and quality of life (QoL) in pediatric patients undergoing high-dose methotrexate (HD-MTX) chemotherapy. Methods: A randomized, controlled, blinded clinical trial was conducted at a tertiary pediatric oncology center, including 21 patients aged 2-17 years receiving HD-MTX protocols. Participants were allocated to IOPBM (660 nm laser) or EOPBM (defocused 810/980 nm laser). OM severity was assessed daily using the World Health Organization scale (grades 0-4), and QoL was evaluated using the OHIP-14 at baseline and at the end of the chemotherapy cycle. Nonparametric tests were applied (p < 0.05). Results: The overall incidence of OM was 90.5%. Grade 4 mucositis occurred only in the IOPBM group, whereas mild to moderate lesions predominated in the EOPBM group. OHIP-14 scores increased significantly at the end of the cycle (p = 0.006), indicating worsening QoL, with higher final scores in the IOPBM group compared to EOPBM (p = 0.046). Conclusion: Neither PBM protocol completely prevented OM; however, EOPBM was associated with lower lesion severity and better preservation of oral health-related QoL, representing a promising and better-tolerated approach in pediatric oncology.
