Resumo: Oral squamous cell papilloma is a benign epithelial proliferation, with well-defined clinical and microscopic features. However, the presence of underlying stromal changes can signal serious systemic conditions. A 51-year-old woman was referred by her dentist to an oral pathology specialist due to an exophytic, keratotic lesion on the tongues dorsum. Following the excisional biopsy, the microscopic examination revealed an amorphous, eosinophilic, compact, and paucicellular matrix beneath the lesion, raising suspicion of amyloid deposits. Congo Red stain demonstrated apple-green birefringence under polarized light, confirming the diagnosis of oral squamous cell papilloma with associated amyloid deposits. The patient was subsequently referred for systemic evaluation. Laboratory studies demonstrated elevated serum kappa light chains. Currently, the patient is under long-term follow-up with a diagnosis of light-chain amyloidosis. This case highlights the critical role of the oral pathologist in identifying subtle histological clues that may lead to a diagnosis of life-threatening systemic diseases.
