Título: A RARE CASE OF SYNCHRONOUS AMELOBLASTOMA AND POORLY DIFFERENTIATED SQUAMOUS CELL CARCINOMA WITH VERRUCAL FEATURES

Resumo: The synchronous occurrence of an odontogenic tumour and an epithelial malignancy is rare and diagnostically challenging. We report a case of a 59-year-old Dusun woman, presented with a progressively enlarging swelling over the left mandible, associated with pain, spontaneous bleeding, and left chin paraesthesia. Radiographic examination revealed a multilocular radiolucent lesion with poorly defined posterior margins. Histopathological evaluation from two sites demonstrated dual pathology, comprising an intraosseous ameloblastomatous epithelial proliferation and an overlying mucosal poorly differentiated malignant squamous proliferation with extensive necrosis and a dysplastic verrucous surface. Immunohistochemically, both components were positive for AE1/AE3 and CK5/6, while CK19 was positive in the ameloblastomatous epithelium. Ki-67 showed high proliferative index in the squamous component and a low index in the ameloblastomatous epithelium. The patient was planned for concurrent chemoradiotherapy but unfortunately succumbed to disease progression. This case underscores the importance of extensive sampling in recognising synchronous lesions with distinct biological behaviour.
