Resumo: Four female patients aged 52-73 years attended an oral cancer prevention unit presenting ulcerated lesions on the left lateral border of the tongue. During anamnesis, three patients denied tobacco and alcohol use, while one reported smoking cessation 25 years earlier and denied alcohol consumption; therefore, all were considered outside the traditional oral cancer risk profile. All patients underwent incisional biopsy, and histopathological analysis confirmed squamous cell carcinoma. Tumor staging ranged from pT3N0 to pT4aN3b. Surgery was the primary treatment modality, including three partial glossectomies and one subtotal glossectomy, combined with unilateral or bilateral neck dissection. Adjuvant radiotherapy was administered to all patients, and concomitant cisplatin-based chemotherapy was performed in two cases. All patients completed treatment. During follow-up, one patient developed cutaneous carcinomatosis and is currently receiving exclusive palliative care; another developed pulmonary disease progression and died, while two remain under follow-up with head and neck and dental care teams.
