Resumo: Uremic stomatitis is a rare condition resulting from the accumulation of urea and nitrogenous compounds in saliva and is typically related to acute or chronic renal failure, which may occur as a complication of multiple myeloma. A 71-year-old male patient presented with multiple painful lesions on the tongue, associated with difficulty eating and weight loss. Intraoral examination revealed white-yellowish plaques and papulo-nodular areas on the dorsum, lateral border, and ventral surface of the tongue, some of which were ulcerated. Laboratory tests showed creatinine 2.83 mg/dL, urea 109.1 mg/dL, eGFR 23 mL/min/1.73 m², hemoglobin 8.3 g/dL, and platelet count 620,000/mm³. Histopathological analysis demonstrated acanthotic epithelium, hydropic degeneration, hyperparakeratosis and a chronic inflammatory infiltrate. A final diagnosis of uremic stomatitis was established based on the clinical and laboratory findings. Further investigation revealed multiple myeloma. Chemotherapy was initiated, resulting in partial improvement in renal function and regression of the oral lesions.
