Título: ULCERATED ORAL LESION IN A PATIENT UNDER TREATMENT FOR SYSTEMIC LUPUS ERYTHEMATOSUS: A CASE REPORT

Resumo: A 40-year-old female patient presented with a painful lesion on the left buccal mucosa, with an evolution of two months. Her medical history was significant for systemic lupus erythematosus, treated with cyclosporine, chloroquine, and ibuprofen. Clinical examination revealed an ulcerated lesion with areas of bleeding. The initial diagnostic hypotheses included oral lupus manifestations, viral infection, and squamous cell carcinoma. Given the observed bleeding, lupus-related hematological alterations were considered, including immune-mediated thrombocytopenia and coagulopathies, associated with lupus anticoagulant or antiphospholipid antibodies. Therefore, laboratory tests were performed prior to biopsy, including blood count, prothrombin time, activated partial thromboplastin time, lupus anticoagulant testing, and viral serology for CMV, HSV-1, HSV-2, and EBV. Results showed elevated IgM levels for HSV-1. These findings confirmed HSV-1 infection, possibly associated with immunosuppressive therapy. The patient was treated with oral acyclovir (200 mg, five times daily) for 14 days. Complete regression of the lesion was observed after treatment.
