Título: Exomic analysis of superficial and invasive front regions in OSCC highlights spatial heterogeneity

Resumo: OBJECTIVE: Intratumoral heterogeneity influences progression and therapeutic resistance in oral squamous cell carcinoma (OSCC). This study investigates spatial genomic divergence between superficial (SUP) and tumor invasive front (TIF) regions using whole-exome sequencing.
STUDY DESIGN: Six treatment-naïve OSCC cases undergoing surgical resection were analysed. Paired SUP and TIF tissues were microdissected and subjected to whole-exome sequencing. Comparative profiling identified shared and region-specific mutations, and heterogeneity indices were computed.
RESULTS: SUP and TIF regions demonstrated distinct yet overlapping mutational landscapes. Core driver mutations (TP53, NOTCH1, EGFR, FAT1) were conserved, while region-specific alterations revealed spatially divergent tumor subclones. SUP harbored 18 unique mutations (including CASP8, HRAS, KRAS), whereas TIF showed 11 unique mutations (notably NFE2L2, TGFBR2, NSD1, KMT2D). Heterogeneity indices varied across cases, with two demonstrating high divergence (>1) and one showing substantial overlap (0.33), reflecting variable clonal evolution patterns.
CONCLUSION: OSCC exhibits clinically relevant spatial genomic heterogeneity with implications for tumor evolution and therapeutic response. Spatially resolved genomic profiling is essential for precision diagnostics and targeted therapy, supporting the emerging paradigm of spatially informed precision oncology in oral cancer.
