Resumo: Mycoplasma pneumoniae (MP) accounts for 4-8% of community-acquired pneumonia, mainly in children and adolescents, with approximately 25% of cases showing extrapulmonary manifestations. We report a 9-year-old boy with recurrent oral lesions initially diagnosed as herpetic gingivostomatitis. Over five years, episodes recurred biannually, more frequently in summer and early autumn, preceded by sore throat, cough, fever, and olfactory changes; seizures were also reported. Management included antivirals and corticosteroids, with two episodes progressing to erythema multiforme requiring hospitalization due to clinical severity. Investigation revealed positive MP IgG and IgM, negative DNA detection, and elevated C-reactive protein; chest X-ray was unremarkable. Treatment with azithromycin and prednisone led to clinical improvement. First described in 2015, MIRM remains underrecognized. This case highlights that MIRM may present exclusively with oral lesions exhibiting a herpetiform pattern and should be considered in the differential diagnosis when clinical features suggest acute herpetic gingivostomatitis or erythema multiforme.
