Título: DENOSUMAB-ASSOCIATED OSTEONECROSIS OF THE JAWS TREATED WITH ADJUVANT LOW-LEVEL LASER THERAPY: A CASE REPORT

Resumo: Osteonecrosis of the jaw is an uncommon but potentially serious complication associated with antiresorptive and antiangiogenic agents. Its etiology is multifactorial, involving systemic, pharmacological, and local factors. A 69-year-old female with a history of breast cancer treated with denosumab, as well as controlled diabetes and hypertension, presented with severe pain after dental extractions, accompanied by bone exposure in both the maxilla and mandible. A diagnosis of stage 2 osteonecrosis of the jaws was established. Conservative management was initiated, consisting of clindamycin therapy, antiseptic irrigation, superficial debridement, and adjunctive low-level laser therapy (650 nm, 1 J/cm² , 6 series of 6 applications each). The clinical course was favorable, albeit prolonged, with progressive pain relief, reduction of exposed bone areas, and the occurrence of atraumatic sequestrectomies. This case underscores the importance of strict systemic control and supports conservative management assisted by low-level laser therapy as an effective, minimally invasive adjuvant therapeutic approach.
