Título: THE FINE LINE BETWEEN AESTHETICS AND PATHOLOGY: ADVERSE REACTION TO HYALURONIC ACID WITH SALIVARY GLAND TUMOR MIMICRY

Resumo: Hyaluronic acid is widely used in aesthetic procedures for facial volume restoration. However, it can cause adverse effects, ranging from local reactions to serious complications such as vascular occlusion and granulomas. The following case depicts a 71-year-old white female patient with erythematous edema of the left buccal mucosa after hyaluronic acid filler injection, which was not initially informed. The intraoral examination revealed irregular soft tissue (20×15×5 mm), whitish with brownish areas and fibrous consistency. Differential diagnoses included foreign body reaction, sarcoidosis, and salivary gland tumor. Transillumination and incisional biopsy were performed, with histopathological analysis showing parakeratinized mucosa and intense granulomatous inflammatory infiltrate with multinucleated giant cells surrounding eosinophilic material. The final diagnosis was chronic granulomatous inflammatory process due to a foreign body. The patient was referred for surgical removal. In this regard, the importance of clinical-histopathological correlation stands out, raising questions about the safety and risks of hyaluronic acid use.
