Resumo: Calcifying odontogenic cysts are rare developmental odontogenic lesions, representing less than 1% of all odontogenic cysts. There are case reports of their association with other odontogenic lesions, particularly compound odontomas.
This case involves a 16-year-old male patient who, during an evaluation for third molar extraction, was found to have a slight increase in volume in the vestibular fornix between teeth 12 and 13. An orthopantomogram revealed a mixed radiolucent lesion, delimited by a radiolucent halo, composed of structures consistent with denticles that were displacing the teeth.
Histopathological examination revealed multiple denticles with organized tissue, as well as a cystic structure lined by odontogenic epithelium, with foci of stellate reticulum, ghost cells oriented toward the lumen, and calcifications. The integration of clinical, imaging, and histopathological findings confirmed the definitive diagnosis of calcifying odontogenic cyst associated with compound odontoma.
