Título: Epithelial-mesenchymal phenotype in a high-grade oral neoplasm: diagnostic and immunohistochemical implications

Resumo: 68-year-old HIV-positive man attends a private consultation reporting a lesion on the tongue. He presents lymphadenopathy in the Ia and Ib lymph nodes. Intraorally, an increase in volume was identified on the left lateral border of the tongue, red in color with yellow areas, firm consistency, defined edges, and an approximate diameter of 7 cm with a duration of six months, as well as painful symptoms. No other significant pathological history. An incisional biopsy of the tongue lesion was performed.The histopathological result was sarcomatoid variant carcinoma; immunohistochemistry studies were performed for diagnostic confirmation: CK-AE1/AE3+, CK5/6+, Vimentin+, P63-/+, CD45, and S-100. The patient was referred to an oncology center. This immunohistochemical profile confirms sarcomatoid carcinoma with epithelial and mesenchymal differentiation, supporting an aggressive biphasic phenotype, relevant for diagnosis and clinical prognosis, highlighting its rarity, diagnostic complexity, value of the comprehensive immunohistochemical panel, therapeutic implications, and the need for timely multidisciplinary management.
