Resumo: Graft-versus-host disease (GVHD) is a complication of bone marrow transplantation (BMT) that is linked to oral lesions and a higher risk of oral squamous cell carcinoma (OSCC).This report describes the follow-up of a 35-year-old male with chronic mucocutaneous GVHD after allogeneic BMT.Multiple lesions were observed in the oral mucosa and lower lip, ranging from indurated nodular erythroleukoplastic lesions on the lip to leukoplakic and erythroleukoplastic lesions on the buccal mucosa and palate, along with inflammatory hyperpigmentation.A lip biopsy confirmed OSCC, and the patient underwent surgical resection with clear margins. Palatal lesions showed hyperkeratosis and interface mucositis. Subsequently, an erythroleukoplastic lesion on the right buccal mucosa that was unresponsive to corticosteroids revealed severe dysplasia. All specimens showed a perilesional lymphocytic inflammatory infiltrate consistent with GVHD-related epithelial damage.This case highlights the importance of regular oral surveillance in GVHD patients to enable the early diagnosis and management of potentially malignant and malignant lesions.
