Resumo: Ameloblastic fibrosarcoma is the malignant counterpart of ameloblastic fibroma, in which only the mesenchymal component exhibits malignant features. A 15-year-old male presented with a four-month history of progressive mobility of teeth 35 and 36, resulting in difficulty chewing. During the last month, the gingiva surrounding these teeth became swollen and painful. Panoramic imaging revealed intraosseous retention of teeth 37 and 38, along with a large radiolucent lesion involving most of the left mandibular body. The basal cortical bone and alveolar ridge appeared thinned and expanded. The lamina dura of teeth 34-36 was absent, with evident root resorption. Radiopaque lines radiated from the cortical bone, and the impacted teeth were displaced toward the basal bone, without a cortical boundary. Histological and immunohistochemical findings demonstrated malignant transformation of the ectomesenchymal component in a mixed odontogenic tumor, arising de novo in the mandible of an adolescent male.
