Título: PRIMARY ORAL MELANOMA ARISING IN AN EDENTULOUS MAXILLARY RIDGE: A DIAGNOSTIC CHALLENGE

Resumo: A 79-year-old woman with hypertension and hypercholesterolemia was referred to Oral Medicine for evaluation of a rapidly evolving pigmented lesion on the right edentulous maxillary ridge. Her medications included risperidone, diazepam, mirtazapine, hydrochlorothiazide, losartan, propranolol, atorvastatin, acetylsalicylic acid, and injectable vitamin B complex. Intraoral examination revealed an irregular, asymmetrical, darkly pigmented lesion with heterogeneous coloration and poorly defined borders, raising suspicion of malignancy. The main diagnostic hypothesis was oral melanoma. An incisional biopsy was promptly performed. Histopathological examination confirmed malignant melanoma. The patient was urgently referred for oncologic management and remains under multidisciplinary follow-up. Oral melanoma is rare and highly aggressive, often diagnosed at advanced stages due to its asymptomatic presentation. This case highlights the critical role of early recognition of atypical pigmented lesions in edentulous areas to improve prognosis and survival outcomes.
