Resumo: 88-year-old male with recurrent squamous cell carcinoma in right parotid gland underwent reirradiation (55 Gy/20 fractions on parotid region and 44 Gy to the ipsilateral lymphatic drainage). From the 10th session onward, developed grade IV radiodermatitis in skin adjacent to irradiated field, not corresponding to the tumor area, characterized by extensive ulceration and exudate. The condition was possibly favored by previous irradiation of the region and aggravated by daily fixation of the dressing with adhesive tape. Concomitantly, a right parotid fistula developed due to tumor necrosis. Clinical evaluation was associated with microbiological culture, revealing growth of Proteus mirabilis and Staphylococcus aureus. final diagnosis was grade IV radiodermatitis associated with reirradiation in skin adjacent to irradiated field, along with a parotid fistula secondary to tumor necrosis. Dental management included dressing adjustment, photobiomodulation, antimicrobial photodynamic therapy, culture-guided antibiotics, resulting healing of the radiodermatitis and complete fistula epithelialization with closure after four months.
