Resumo: Oral leukoplakia (OL) is the most common oral potentially malignant disorder and is frequently managed with CO2 laser ablation. However, alternative technologies such as the thulium fiber laser (TFL) remain underexplored, with limited data regarding their efficacy, safety, and tolerability in OL management. We present a case series of 17 patients (39 lesions; median age 66 years [50-81]; 41% smokers) with histological diagnoses including nonreactive hyperkeratosis (26%), low-grade dysplasia (56%), and high-grade dysplasia (18%), all treated with TFL. Post-operative adverse events, assessed at a mean first follow-up of 30±20 days, included pain (59%), swelling (13%), osteonecrosis (10%), dysphagia (8%), pyogenic granulomas (8%), and bleeding (3%). At an average follow-up of 60±62 days, one lesion underwent malignant transformation, with three residual lesions and one recurrence observed. Comparative studies with other lasers are warranted to better define the risk-benefit profile of TFL.
