Resumo: OBJECTIVE: To reflect on the limitations of telestomatology for diagnosing oral potentially malignant disorders (OPMD) within the Brazilian Unified Health System (SUS). The study emphasizes the indispensability of in-person clinical examination and the stomatologist, from the specialists role, aligned with current guidelines for early oral cancer detection.
STUDY DESIGN: This is a reflective theoretical essay based on a critical analysis of scientific literature and current regulatory frameworks. It addresses traditional stomatological semiology, the limitations of remote assessment, and the strategic role of Primary Health Care (PHC) as the organizing axis of the oral health care network.
RESULTS: Telestomatology presents critical limitations, mainly due to the inability to perform palpation and the loss of depth perception (three-dimensionality), which may compromise the identification of early, subtle lesions - such as a thin, smooth, leukoplakia or minimal erythroplakia, for example. A safe diagnosis remains dependent on the primary care dentist, who must be proficient in early recognition and timely referral, adhering to established clinical protocols to ensure patient safety.
CONCLUSION: Telestomatology should be regarded as a complementary tool rather than a substitute for in-person examination. The stomatologist maintains a central essential to ensure the effectiveness, safety, and integrality of care within the SUS.
