Resumo: A nine-year-old patient presented with an intraoral fistula and facial abscess. The patient had a recent history of antibiotic treatment. Clinical evaluation, dental panoramic radiography and cone-beam tomography diagnosed an abscess with an intraoral fistula associated with tooth 12. The tooth exhibited a "dens in dente" malformation and was deemed unviable for dental function. Extraction was the treatment of choice to eliminate the focus of infection. After psychological conditioning, the patient received treatment in an outpatient setting. At the seven-day post-operative evaluation, the intraoral fistula had resolved. The extracted tooth was sent to the Pathology Service for evaluation via SEM (Scanning Electron Microscopy), providing a detailed study of the "Dens in Dente" dental structures. The patient remains in follow-up, having completed one year without recurrent infectious alterations, and was referred for evaluation and treatment in the areas of pediatric dentistry and orthodontics.
