Título: Light-chain-restricted Russell body labial sialadenitis in Sjögrens disease patient: an immunohistochemical study

Resumo: Several intraoral chronic inflammatory conditions, such as periapical lesions, dental pulpitis, periodontal disease, peri-implant disease, and plasma cell mucositis, can present plasma cell infiltration admixed with Russell bodies (RBs), as well as syphilis and B-cell pseudolymphoma. Recently, we reported an extremely rare case of light-chain-restricted RB intraoral mucositis. In our experience, when microscopically assessing minor salivary glands (mSGs) in Sjögrens disease (SD), the presence of RBs in these specimens is uncommon. A 28-year-old woman with SD diagnosis and complaining of xerostomia was referred for mSG biopsy. Microscopy revealed features of focal lymphocytic sialadenitis; however, unusually, numerous RBs were visualized admixed with lymphoid aggregates. The focus score was 0.9. Immunohistochemistry showed a similar proportion of CD20+ and CD3+ cells, whereas CD138 highlighted RBs/plasma cells, the same ones with IgG lambda restriction. The current case shows that RB sialadenitis can be observed in SD patients; here, we present the second case report.
