Resumo: Alveolar soft part sarcoma is a neoplasm of uncertain histogenesis, classified among ultra-rare sarcomas and characterized by the ASPSCR1-TFE3 gene fusion. A 7-year-old female presented with an asymptomatic erythematous tongue lesion with progressive growth over a few months. An incisional biopsy yielded an initial histopathological diagnosis of paraganglioma, and the case was referred for revision. Histopathological examination showed nests of polygonal tumor cells with well-defined borders, abundant eosinophilic to clear cytoplasm, and large nuclei, separated by fibrous septa and associated with a prominent vascular network, with focal vascular invasion. Tumor cells were negative for CK7, S100, and AE1/AE3, with nonspecific SMA staining. CD34 highlighted the vascular component. PAS-diastase demonstrated cytoplasmic granules, and TFE3 showed strong nuclear positivity. The diagnosis was alveolar soft part sarcoma, and the patient was referred for oncologic management. This case underscores the importance of integrating morphology and immunohistochemistry in diagnosing this rare pediatric tumor.
