Resumo: Female patient of 27 year-old, shows a volume increase in back of the hard palate, its oval in shape, smooth surface, pink color, firm consistency, sessile base of 3 x 2 cm in diameter. It was an incisional biopsy and routine staining showed a neoplastic lesion characterized for proliferation of epithelioid cells arranged in a mixed pattern of nests, islands and plexuses, interspersed with a sparse hyaline stroma and loose fibrous connective tissue, medium caliber ductal structures with protease content and multiple keratin cyst. Immunohistochimicals stains were made. The following results were obtained: calponin positive, CK7 positive, S100 positive, Ki-67 positive >5%. A final diagnosis of pleomorphic adenoma with squamous differentiation was established and the patient was referred to his hospital for monitoring and follow-up. This case highlights the atypical presentation of squamous differentiation in pleomorphic adenoma, which is not the usual presentation.
