Resumo: Metastases to the oral cavity are rare, representing less than 1% of oral malignancies. This report describes a 24-year-old Caucasian male with morbid obesity (BMI 49.9) and a history of metastatic germ cell tumor involving the lumbar spine, skull, and lungs. He was admitted to the intensive care unit with respiratory failure and uncontrolled pain. Oral examination revealed a rapidly growing mass in the lower third molar region, presenting with friable consistency, ulcerated surface, painless characteristics, and no spontaneous bleeding. Given the patient's oncologic history and the lesion's aggressive features, an incisional biopsy was performed. Histopathological findings confirmed oral metastasis of the germ cell tumor, indicating further systemic dissemination. Chemotherapy was contraindicated due to an infected sacral pressure ulcer and high risk of sepsis. Therefore, palliative care was initiated. This case emphasizes the importance of thorough oral evaluation and continuous dental follow-up in patients with advanced malignancies.
