Título: CD30 IMMUNOHISTOCHEMICAL EXPRESSION IN PLASMABLASTIC LYMPHOMAS

Resumo: Objective: To assess the CD30 immunohistochemical (IHC) expression in plasmablastic lymphoma (PBL) and determine if patients might benefit from Brentuximab Vedotin (BV) treatment.
Study design: A retrospective review of histological reports and slides was conducted at a tertiary health center over 13 years (2011-2023). Tissue blocks with adequate material were selected for CD30 (JCM182) IHC staining. CD30 IHC expression in tumor cells was categorized as negative or positive. Positive staining was semiquantitatively evaluated by assessing percentage of tumor cells on a 5-tier scale: 0= 1-5%; 1= 6-24%; 2= 25-49%; 3= 50-75%; 4= >75%. The expression (CD30+ vs. CD30-) and scores (0-4) were correlated with HIV and Epstein-Barr virus encoded RNA in-situ hybridization (EBER-ISH) status.
Results: Cases included 87 PBL with a mean age of 44,17 (SD=11,6) and a male predominance of 64 (73,6%). Patients included 62 (71,3%) who were HIV+ and 85 (97,7%) cases of EBER-ISH+. Of these, 32 (36,8%) were CD30+. There was a positive correlation between CD30+ IHC expression with HIV+ and EBER-ISH+ status.
Conclusion: These results suggests that HIV and EBV infections significantly influence CD30 expression in PBL. Although limited, routine CD30 IHC staining in PBL diagnosis may help select patients for BV treatment to improve prognosis.
