Título: Excisional biopsy enhances clinical-histopathological diagnostic concordance

Resumo: Objective: The concordance rate between diagnostic hypotheses (DH) and final diagnoses (FD) is moderate, reflecting the challenges in accurately identifying oral lesions based solely on clinical examination. Here, we evaluated the concordance level between the DH and FD in a national and international reference oral pathology laboratory.
Study design: A total of 2,054 cases were included in the analysis. The data were inserted in a data bank, containing age, sex, type of biopsy (excisional, incisional), DH, and FD. Concordance was defined in those cases where at least one DH was compatible with the FD. The associations between the outcome variable (concordance) and the dependent variables (sex, age, and type of biopsy) were evaluated using the chi-square test.
Results: A slightly higher representation of female patients (54,09%) among individuals undergoing biopsy procedures was observed. Excisional biopsy was the most frequent procedure (64.65%). No statistically significant difference was found according to sex and age. In contrast, excisional biopsy showed a statistically significant association with diagnostic concordance, indicating that the biopsy technique may influence the likelihood of clinical-histopathological agreement.
Conclusion: These findings highlight the importance of appropriate biopsy technique in improving clinical-histopathological correlation and supporting accurate diagnostic outcomes in oral pathology.
