Resumo: To report a case of recurrent Medication-related Osteonecrosis of the Jaw (MRONJ) managed with surgical intervention combined with an active oxygen gel as an adjunct to wound healing. A 77-year-old female with a history of breast cancer and bone metastasis, previously treated with bisphosphonates, presented with stage 2 MRONJ following facial trauma. Intraoral examination revealed bone exposure in the right maxillary alveolar ridge and tooth mobility on the left side. Surgical debridement with Platelet-Rich Fibrin (L-PRF) was performed, followed by extraction of tooth 23. Subsequently, a new stage 2 MRONJ lesion developed on the left side and was treated with surgery associated with topical oxygen gel (blue®m). After three surgical procedures, no clinical signs of infection were observed, and complete mucosal healing was achieved. The combined approach was associated with favorable clinical outcomes, suggesting a potential adjunctive role of oxygen-based gels in the management of recurrent MRONJ cases in practice.
