Título: RADIOMICS IN CBCT AS A PREDICTOR OF PERIAPICAL BONE CHANGES IN MOLARS WITH VERTICAL DEFECTS AND FURCATION LESIONS

Resumo: Objective: To investigate possible bone impairments in the apical region of maxillary first molars affected by furcation lesions through texture analysis (TA) applied to cone-beam computed tomography (CBCT) images, evaluating whether the progression of periodontal bone loss is associated with structural alterations in periapical bone. Study design: Analytical diagnostic study with 120 maxillary first molars divided into four groups (n=30): control (CG), cervical bone loss (CeG), middle (MG), and apical (AG). CBCT images were analyzed in OnDemand3D for lesion classification and exported to MaZda 3.20, where a standardized circular ROI (3.0×3.0 mm) was placed in the periapical region of each root. Eleven gray-level co-occurrence matrix (GLCM) parameters were calculated. Statistical analysis employed Kruskal-Wallis and Dunn tests with Bonferroni correction. Results: The mesiobuccal root showed the greatest number of parameters with significant differences between groups S(1,0) Correlat, SumOfSqs, SumAverg, SumVarnc, SumEntrp, Entropy, and DifEntrp), with MG presenting higher values than CG. In the palatal root, only S(3,3) Correlat was significant; in the distobuccal root, only SumAverg (p<0.001). Conclusion: TA applied to CBCT images detected periapical microstructural bone alterations in molars with furcation lesions, particularly in the mesiobuccal root, even in the absence of visible radiographic changes.
