Resumo: Oral potentially malignant disorders, such as leukoplakia, carry an increased risk of neoplastic transformation, with smoking being the primary etiological factor. A 53-year-old male patient, hypertensive and smoker (one pack/day), presented in 2020 with extensive, non-scrapable white plaques on the floor of the mouth, ventral tongue, and bilateral buccal mucosa. The clinical diagnosis was leukoplakia. After counseling regarding the risk of malignant transformation and its direct correlation with smoking, the patient ceased the habit in 2023. At the 2025 clinical follow-up, a significant involution of the lesions was observed, with complete remission in the buccal mucosa and a favorable clinical appearance on the floor of the mouth, showing a negative result on tissue transillumination. Clinical management focused on causal factor elimination and rigorous follow-up proved effective, highlighting the importance of behavioral intervention in managing smoking-related oral lesions. The patient remains under periodic monitoring.
