Título: Beyond The Usual: A Rare Case of Ameloblastoma with Osteoplasia

Resumo: Ameloblastoma is a benign but locally aggressive epithelial odontogenic tumor of the jaws. Prominent osseous formation is an uncommon finding in ameloblastoma. Although some reports have referred to this pattern as osteoplastic ameloblastoma, it is not an established histological subtype and appears more frequently associated with desmoplastic ameloblastoma. We report a case of a 30-year-old female with ameloblastoma exhibiting marked osteoplasia. The lesion presented as a slow-growing swelling of the right mandible, extending from the incisor to the premolar region with buccolingual expansion for over 10 years. Radiographs showed a honeycomb appearance. Incisional biopsy demonstrated ameloblastoma with desmoplastic features, with CK19 and beta-catenin positivity, low Ki-67 <1%, and equivocal BRAFV6000E immunohistochemistry. Surgical resection revealed prominent osteoplasia, with woven-to-lamellar bone and ameloblastomatous islands within desmoplastic stroma. There has no recurrence after one year postoperatively. This case highlights the diagnostic challenge of osseous formation in ameloblastoma, suggesting tumour-induced osteogenesis and stromal induction.
