Título: A DIAGNOSTIC CHALLENGE OF ORAL ULCERS AS THE FIRST MANIFESTATION OF SECONDARY SYPHILIS - CASE REPORT

Resumo: A 25-year-old male patient was referred for evaluation of intraoral ulcers with two months of evaluation that had progressively worsened. His medical history revealed episodes of anxiety. Furthermore, the patient denied risky sexual behavior. Extraoral examination revealed submental lymphadenopathy and the absence of skin lesions. Intraoral examination showed bilateral whitish, well-defined, and hardened ulcers at the labial commissure, buccal mucosa, and lateral border of the tongue. An incisional biopsy of the buccal mucosa was performed and showed epithelium with exocytosis and intense, predominantly plasmocytic inflammatory infiltrate. Immunohistochemical analysis for Treponema pallidum was positive. Additionally, the labs showed neutropenia and reactivity for VDRL and FTA-ABS tests. Thus, the diagnosis of syphilis was confirmed. The patient was referred for medical treatment with benzathine penicillin G, resulting in subsequent improvement of the lesions.
