Título: ORAL MUCORMYCOSIS IN A PATIENT WITH ACUTE MYELOID LEUKEMIA: DIAGNOSTIC CHALLENGE AND CLINICOPATHOLOGICAL CORRELATION

Resumo: A 70-year-old female patient with a diagnosis of acute myeloid leukemia, undergoing her third cycle of chemotherapy, developed prolonged neutropenia during treatment. During this period, she presented with severe pain associated with a rapidly progressing lesion in the region of the lower second molar and retromolar trigone. Despite systemic antibiotic therapy, there was significant worsening of the lesion within 48 hours, with progression to a violaceous appearance, facial swelling, trismus and desquamative gingivitis. Given the clinical presentation, diagnostic hypotheses included mucormycosis, leukemic infiltration, and other fungal infections. Fine-needle aspiration was performed, followed by an incisional biopsy, which confirmed mucormycosis. Oral mucormycosis typically shows a predilection for the maxilla, with mandibular involvement being extremely rare, which underscores the uniqueness of this case. This report highlights the aggressive course and high mortality risk of mucormycosis in immunocompromised patients, as well as the importance of early diagnosis and diagnostic confirmation in atypical lesions.
