Título: INTEGRATIVE TRANSCRIPTOMIC ANALYSIS IDENTIFIES PROGNOSTIC BIOMARKERS AND THERAPEUTIC TARGETS IN ORAL CANCER

Resumo: Objective: To identify differentially expressed genes in oral squamous cell carcinoma (OSCC) and evaluate their potential as prognostic biomarkers associated with tumor progression and metastatic behavior.
Study Design: Transcriptomic data from 313 TCGA-HNSC samples were analyzed. Differential gene expression was assessed across tumor, adjacent margin, and stromal compartments. Functional enrichment and pathway analyses were performed to identify biological processes linked to disease progression and clinical outcomes.
Results: Downregulated genes were primarily associated with metabolic pathways, whereas upregulated genes were enriched in oncogenic signaling, extracellular matrix (ECM) remodeling, cell adhesion, and HPV-related pathways. Gene expression profiles indicated activation of proliferative basal and progenitor-like cell populations. Notably, 81 proteins were exclusively upregulated in lymph node-positive (N+) cases, highlighting a molecular signature associated with metastatic disease. These proteins were linked to chromatin organization, cytoskeletal dynamics, and regulation of cell death. Key enriched pathways included focal adhesion, PI3K-Akt signaling, ferroptosis, and inflammation-related processes.
Conclusion: The identified gene signatures demonstrate strong potential as prognostic biomarkers for OSCC, particularly in predicting metastatic risk. Pathways related to ECM remodeling, PI3K-Akt signaling, and ferroptosis represent promising targets for patient stratification and may contribute to improved prognostic assessment and personalized therapeutic strategies.
