Resumo: Objective: This systematic review aimed to evaluate the effect of low-level laser therapy (LLLT) in the management of secondary trismus (ST) following head and neck cancer (HNC) treatment. Study Design: The review followed the PRISMA guidelines. Searches were conducted in 2025 across five databases, supplemented by manual screening and gray literature. The risk of bias was also assessed. Results: A total of 158 records were identified. After applying the eligibility criteria, four studies were included, comprising 142 patients, with a mean age of 59.4 years. Two studies presented a moderate risk of bias, and two showed a severe risk. A significant increase in maximal mouth opening (MMO) was observed across all studies in the LLLT group [baseline MMO: 18.16 ± 4.70 mm; post-treatment MMO: 33.06 ± 9.51 mm; mean gain: 11.51 ± 6.35 mm; p < 0.005]. Statistically significant improvements were also observed in pain intensity and quality of life. Conclusion: Our findings suggested that LLLT presents positive effects in the management of ST after HNC treatment, particularly when associated with other therapies, improving MMO, pain relief and increasing quality of life. Further studies are needed to clarify the role of LLLT as single therapy in ST management.
