Título: GLANDULAR ODONTOGENIC CYST OF THE ANTERIOR MANDIBLE: A RARE ODONTOGENIC CYST

Resumo: The glandular odontogenic cyst (GOC) constitutes an exceedingly rare developmental odontogenic lesion and persists as a significant diagnostic pitfall owing to its pronounced clinicopathological overlap with other odontogenic entities. Herein, a 56-year-old male presented with a painless swelling in the anterior mandibular region. Computed tomography revealed an expansive, multiseptated cystic lesion within the left paramedian mandibular body (4.1 × 4.0 × 2.9 cm), associated with adjacent dental root involvement, cortical thinning, and focal perforation. Histopathological examination demonstrated a multilocular cyst lined by epithelium of variable thickness (2-10 cell layers), comprising mucous and ciliated columnar cells, microcystic or duct-like structures, and papillary projections within a fibrous capsule. In accordance with the criteria proposed by Fowler et al. (2011), eight diagnostic features were fulfilled, whereas two were absent or inconspicuous. Accordingly, a definitive diagnosis of GOC was established via incisional biopsy, and the patient is currently undergoing definitive surgical intervention.
