Título: FOREGUT CYST: A CASE REPORT AND A RARE DIFFERENTIAL DIAGNOSIS FOR SUBMANDIBULAR MASSES

Resumo: Foregut cysts (FC) are uncommon hamartomas in the head and neck region; when they do occur, the oral cavity is most frequently affected. This report describes a 50-year-old male patient who reported the appearance of a nodular lesion in the left cervical region since 2014, with progressive increase in size and discomfort during swallowing, but no pain or other complaints. Cervical ultrasonography revealed a cystic lesion in levels IB and IIA, with well defined margins, anechoic, and no flow on Doppler, suggestive of a branchial cyst. After surgical excision, macroscopic examination identified a well vascularized cystic lesion measuring 8.0x6.0x4.0cm, filled with gelatinous fluid. Microscopically, a fibrous capsule was observed, exhibiting collagen fibers, numerous blood vessels, and smooth muscle cells (SMA+), along with a predominantly columnar epithelial lining rich in mucous cells (PAS+, CK7+, CK20-), findings suggestive of FC. Treatment consists of the excision itself, which is both diagnostic and therapeutic.
