Título: Intraosseous undifferentiated sarcoma of the mandible: A challenging clinicopathological case

Resumo: Undifferentiated sarcoma represents a diagnosis of exclusion. The present report describes an 18-year-old male patient referred to a tertiary hospital due to a comminuted fracture of left hemimandible after a sports trauma. Radiographic examination revealed an extensive fracture with ill-defined borders and absence of cortical bone associated with the lesion. Incisional biopsy showed a proliferation of spindle cells with cellular and nuclear pleomorphism, atypical mitoses, and positivity exclusively for vimentin and SOX-9, with exclusion of other lineages such as neural and epithelial. A similar profile was observed upon analysis of the hemimandibulectomy specimen, where the cell proliferation index was established at 20-30% by Ki 67 and staged as pT1N0. Therefore, negativity for the remaining 27 antibodies tested, including epigenetic investigation of H3K27 trimethylation with preserved expression, corroborated the interpretation of undifferentiated sarcoma, making a worse prognosis less favorable. The patient is currently under follow-up at the reference oncology hospital.
