Resumo: Oral pigmentation is most commonly physiological in nature, resulting mainly from melanin; however, pigmentation generated by exogenous factors, such as embedded foreign material, dental restoration amalgam, or pencil, is referred to as "oral tattoo". Noteworthy, the occurrence of titanium tattoo or peri-implant pigmentation associated with zirconia components, presenting as clinically blue-gray macules similar to amalgam or graphite tattoos, represent a diagnostic challenge. To date, about 11 cases of intraoral graphite tattoo have been reported. A 55-year-old woman was referred for examination of a pigmented lesion on the floor of the mouth, which was detected by the dental surgeon who was performing the prosthetic restoration on dental implants. Microscopy revealed multiple deposits of brownish granular exogenous material inducing a foreign body-type granulomatous response. An incineration method was performed, observing that the exogenous material disappeared, confirming the diagnosis of graphite tattooing.
