Resumo: Osteosarcoma is a rare malignant bone tumor and the most common primary sarcoma of the oral cavity, characterized by immature bone production. Diagnosis can be challenging, especially in cases with minimal osteoid. A 62-year-old male presented with mandibular swelling and paresthesia. Panoramic radiography showed a radiolucent lesion from the right canine to the second molar, with root resorption of the first premolar. Incisional biopsy revealed atypical spindle to ovoid cells in a myxoid to fibrous stroma, with focal osteoid formation. Tumor cells exhibit mild pleomorphism and rare atypical mitoses. Immunohistochemistry was positive for vimentin, HHF35, SMA, CD99, CDK4, MDM2, SATB2 and SOX9, with focal S100, and negative for AE1/AE3, desmin, calponin, H-caldesmon, and myogenin. Ki-67 was approximately 15%. Integration of clinical, radiographic, histopathological, and immunohistochemical findings confirmed the diagnosis of dedifferentiated low grade central osteossarcoma. The patient had pulmonary and hepatic metastases and was managed with palliative care.
